My mom has adenomyosis, with endometrial polyp. She has severe pain and bleeding during her periods. She's anaemic with a Hb count of 8 and a TSH level of 8. What management would you suggest - medical or surgical? And in either choices pls specify specifically.

Usually at 50 yrs of age , due to anovulatory cycles, there will not be pain during menstruation. But due to adenomyosis, having severe pain and heavy bleeding which resulted in anemia.
Correct your hypothyroid status and anemia first and USG also shows thickened endometrium and polyp which shows hyperestrogenic state.
Get consultation for endometrial biopsy and based on report the treatment varies.
At 50 yrs , the thickness is quite high and better to do biopsy and plan accordingly.
Medical treatment can be tried but not too long if no improvement. Surgical option would be better otherwise
The polyp needs surgical removal
After the removal of the polyp the patient can be put on hormones to control her bleeding
For hypothyroidism she needs a medication
For anaemia she needs iron sucrose injections followed by oral therapy
