Abdominal bloating
Hello Doctor, I have been experiencing abdominal bloating, especially at night while lying down. I also feel a sensation of tightness/pressure in my abdomen. I often feel like I need to pass gas, but when I try, I am usually unable to release it. Sometimes I stop trying because I am afraid that stool might come out along with the gas. Could this be due to trapped gas, constipation, IBS, or any other digestive problem? Please advise what could be the possible cause and whether I should take any medicine or follow any specific diet/remedy. Thank you.

At your age, with no other illness, this pattern of bloating, tightness and difficulty passing gas is most often functional gas and bloating, often overlapping with mild constipation or irritable bowel syndrome. Habits matter a lot here: eating too fast, aerated drinks, chewing gum, skipping meals, excess tea/coffee, very high dairy or wheat load, and stress all worsen it. Try smaller regular meals, adequate water, 20-30 minutes of daily walking, and a walk after dinner instead of lying down immediately. Reduce carbonated drinks, deep fried food, maida and excess milk for two weeks and see if the pattern improves. Fear of leakage while passing gas is common and usually settles once constipation is corrected with fibre and regular bowel habit.

Give the diet and lifestyle changes 2-3 weeks. If bloating persists, get a basic check — CBC, thyroid, blood sugar, coeliac screen and an ultrasound abdomen — before starting any regular medicine. You have not stated about Duration of complaint Constipation or diarrhea type stool Your dietary preference.veg/nonveg How much fruit and vegetables you include in diet Appetite and sleep issues All are important before one can say about IBS Yes excessive flatulence and feeling stool will come with that means incomplete evacuation for long Any Liver issues need to be evaluated

Avoid going to the bed immediately after the dinner.Change your lifestyle, include physical activity atleast for 30min a day.It could be due to dyspepsia, flatulence.Try drotin plus twice daily and ganaton twice daily for 5 days

If your daily work or study is related to sitting whole day or more than 4 hours so you can avoid it or make gaps between your sitting period .

After dinner follow these rules :- 1)Do not immediately lie down after dinner. 2)Instead, go for a walk for atleast 30 minutes. 3)Do not immediately drink water after the last bite of your dinner as it will dilute the digestive acids in your stomach and delay digestion leading to gas accumulation and belly heaviness. 4)Instead, when you return from your 30 minutes walk have a glass of warm water.

You should use warm water ealry in morning and take pantoprazole and you should gap the minimum 6 hours between the meals you should take and take night meal as early as possible and after meal walk 15-20 minute brisk walk .

Advise adequate hydration, regular physical activity, high-fibre diet if tolerated, and avoid gas-producing foods/carbonated drinks. If persistent or recurrent, an in-person evaluation is recommended to assess bowel habits and rule out other causes. Seek urgent care if severe pain, persistent vomiting, abdominal distension, blood in stool, or inability to pass stool/gas occurs.”

Your night-time bloating, abdominal tightness and urge to pass gas without being able to release it, with fear of stool leakage, most commonly happens due to constipation with trapped gas, IBS, or uncoordinated pelvic floor muscles where the muscles tighten instead of relaxing, often worsened by gas-producing foods like onion, garlic, beans, cabbage, milk, carbonated drinks, eating fast and lying down soon after dinner; try regular small meals eaten slowly, avoid those gas-forming foods and carbonated drinks for a few days, drink 2-2.5L water, walk 15-20 min after dinner and avoid lying down immediately, try left-side lying or knee-to-chest position to release gas, and keep a fixed toilet time with a footstool without straining, and especially if you have blood in stool, weight loss, fever, severe pain or actual stool leakage consult me on practo.

"What could be the reason?" may be a difficult question to answer. It may be easier to manage this problem. And here too we have to remember that it can be managed by lifestyle modifications and not by popping in a pill or two. Hello! This pattern usually happens when trapped gas gets backed up due to mild constipation or incomplete bowel clearing..

Symptoms are suggestive of functional bloating, constipation, or IBS, but IBS requires recurrent abdominal pain with altered bowel habits. Difficulty passing flatus should be assessed along with bowel movements and other symptoms.

Have a very light dinner before 7 pm every night.avoid rice potatoes and other starchy food.dont eat fruits after dinner.it’ll ferment and cause gas.if bloating is too much simethicone tablets will help in absorbing gas..walk after dinner.for 15 to 20mts.light walk .pass motion everyday in the morning at a fixed time

Many problems in gut can cause bloating ranging from stomach to intestines. Check what food is causing bloating? After how many minutes of eating u experience it? Depending upon these we get to know what foods can u eliminate temporarily, so that u will recover.

I would say dont run after medications, like all the other doctors told u life style modification is key here. For example - eat good clean food/avoid junk food. No food 2 hours before food .
