The Urogynecology and Pelvic Health Institute (UPHI) is the premier center in India dedicated to the treatment of prolapse, bladder and bowel control problems. UPHI is a first of its kind, tertiary, clinical and academic international centre of excellence providing multidisciplinary care in the field of urogynecology and pelvic reconstructive surgery in India.
UPHI is founded by leading urogynecologists, Dr. Aparna Hegde and Dr. Sanjay Gandhi, who have trained in some of the most prestigious universities of the world such as Stanford University, Cleveland Clinic Florida and Northwestern University. Dr. Aparna Hegde has returned back to India after six years of education in the US to establish UPHI and is proud to bring back the latest treatments available internationally, including many that are new to the country. Dr. Sanjay Gandhi, our International Director and visiting urogynecologist, is committed to help Dr. Hegde ensure that UPHI always keeps abreast of the latest developments in the field.
The conditions we treat include prolapse (sagging of pelvic organs), urinary incontinence (leakage), fecal incontinence, other bladder and bowel control problems including urgency and frequency of urination, bladder emptying problems, chronic constipation, recurrent urinary tract infections, hematuria, pelvic pain, sexual dysfunction, neuro-urological problems, fistulas, mesh complications, vaginal agenesis etc.

UPHI has many firsts to its credit:
• Introduction of the concept of integrated care of the pelvic floor: Urogynecology is a super specialty of gynecology that recognizes
that the pelvic floor is one and not artificially compartmentalized. UPHI is introducing for the first time the concept of integrated
care of the pelvic floor where our urogynecologist and colorectal physician will work hand-in-hand to provide the most
comprehensive, integrated and multidisciplinary pelvic floor diagnosis and treatment, including all conservative and surgical
solutions, under one roof
• The most comprehensive and advanced diagnostics available in the field including some which we are introducing for the first time
in the country such as
- Multicompartment 2D and 3D ultrasound imaging of the pelvic floor for the imaging of the mesh tape following sling surgery for
stress urinary incontinence, meshes used in prolapse surgery, pelvic floor damage caused by childbirth and bladder, urethral and
anorectal pathologies. This formed a major part of Dr. Hegde’s research during her fellowship at Cleveland Clinic Florida and many
of the protocols she designed are now being implemented in Cleveland Clinic and other US universities. She has been part of the
faculty at the ultrasound workshops of International Urogynecological Association (IUGA) teaching this new skill and protocols.
- 3D endoanal ultrasound (for imaging of anal sphincters) and anal manometry procedure that measures the pressure and strength
of the anorectal muscles, thereby helping to diagnose problems of bowel empyting and control.
- Comprehensive Urodynamics: We are introducing air-charged catheters with microtransducers in place of the water-perfused ones
used elsewhere in the country. Air-charged catheters have certain advantages over water-perfused ones: they do not require
recalibration during the procedure, provide circumferential pressure sensing for most accurate readings and the air column is
relatively weightless and thus not affected by water pressure errors.
• A comprehensive physiotherapy and behavioral modification program to offer treatment options other than surgery.
Physiotherapy of the pelvic floor including techniques like electrical stimulation and biofeedback, play a major role in providing
relief to a patient with urgency incontinence, pelvic pain, chronic constipation and pelvic floor dyssyenergia (condition in which
there is paradoxical contraction of the pelvic floor instead of relaxation).
• Newer pessaries: UPHI is introducing for the first time in India Gellhorn, cube etc pessaries, which were hitherto unavailable in
India, to provide support for prolapse of stage 2 and beyond.
• Focus on chronic care: UPHI will create a special cadre of nurse continence specialists to provide chronic care for patients with
bladder and bowel problems resulting from neurological issues.
• Sacral neuromodulation: UPHI is one of the first institutions in the country to introduce this new therapy which alters the function
of the sacral nerves supplying the bladder to treat patients with urgency incontinence, chronic urinary retention and fecal leakage.
Dr. Hegde underwent training in the procedure during her fellowship in the US.
With the introduction of these new technologies and treatments, both surgical and non-surgical, UPHI aims to provide the most comprehensive treatments available world-wide to the women with urogynecological problems.
