Xray. Periodontal issue in 46,47
I have constant pain in jaw under ear and neck for 15 months now. I have constant tapping and biting pressure on tooth 46. I went to dentist for jaw pain under ear and he spotted tooth 46 issue and sent me to endodontist. I didn't have any tooth pain before this visit. Only severe jaw pain under ear and neck. Endo couldn't see any fracture or crack in root. He said to do root canal for this tapping/biting pressure on it. Had root canal in September and still no changes. Don't know what's wrong. Checked tmj with MRI and CT. Tmj normal. OMFS said Myofascial pain and sent me to PT. I saw another Endodontist for second opinion. took x-rays and said I have pdl widening on 47. And apical periodontitis in 46. He said I have high hitting spot in 47(no dental work on this tooth) and wanted me to polish that down. I couldn't feel the bite at all and it feels different and upper molars hitting hard and tight on this side. I have tight jaw, neck,back and shoulder muscles.how to treat this tooth?

You should rule out spondylitis. If there is a habit/history of clenching/grinding teeth at night, a night guard should be tried for that. The 46,47 symptoms may be unrelated to the jaw neck pain.
