Warfarin patient
Patient using warfarin tablet of 3mg .now it's ptnir test report come 1.27 so should I increase the dose of warfarin in 5 mg or not .she is heart pateint

Target INR is at least 2 ideally between 2 to 3. Increase warfarin dosage to 4 mg once a day ( 5 pm,  to avoid interactions with major meals) as INR target is not achieved. Check INR after 5  days. She is 75 years old so it's important to know for which heart condition she is taking warfarin.
