It gave temporary relief but severe pain is still existing while gulping food and water also getting ear pain associated with this

Treatment
Betadine gargle add 3ml solution in hot to warm water gargle it before meals two times for 3to5.
Tab cefexim o200 two times after food for five days.
Zerodol sp3 times a day after food for five days. Montek lc one at night for five days after dinner.
Plenty of water n liquid luck warm. Home made fresh semiliquid diet.

• Betadine gargle or Chlorhexidine mouthwash (Hexidine) — 2–3 times/day (diluted as per bottle)
• Soothing remedies:
• Sip warm water + honey + few drops lemon frequently
• Chew ice cubes or take cold milk/ice cream slowly — numbs throat temporarily
• Throat lozenges (Strepsils, Vicks, Dorithricin) — every 2–3 hours
• Diet & rest:
• Only soft, cool foods (khichdi, curd rice, dal-rice, banana, soup, idli)
• Avoid spicy, hot, sour, fried food completely
• Speak less, rest voice
• Sleep with head slightly elevated (2 pillows)
Severe pain on swallowing after azithromycin usually means you need stronger/longer antibiotic + symptomatic relief.
Please see a doctor today/tomorrow — they will most likely give a 7–10 day course of amoxicillin or similar + pain control.

Azee is for atypical, looks like typical bacterial inf, aminopenicillin would suffice ,consult and give detailed history, would prescribe some meds,you'll be fine within 3days,
Pharyngitis with referred ear pain via shared nerve supply.but its doesn't mean that infection occure in ear also. You have viral infection that's why azithromycin antibiotic is not sensitive fully to control your symptoms, its reversible and self resolving in few days.Warm saline gargles 3–4 times daily.Maintain good hydration and soft diet.please feel free to consult me on practo for further management and evaluation.
