In a Grade 3 varicocele, the veins are visibly and palpably dilated, so it is important to distinguish between symptom control and correction of the underlying venous abnormality. At present, there is no good-quality clinical evidence that Ayurveda or Homeopathy can permanently close/reverse the dilated veins of a Grade 3 varicocele. They may potentially provide supportive relief of discomfort or improve general wellbeing, but they should not be presented as an alternative to definitive varicocele treatment when there is a clear indication for intervention. There is substantially better evidence for varicocele repair in appropriately selected patients. Current urological guidelines support considering repair particularly in men with a palpable clinical varicocele, infertility and abnormal semen parameters.

The decision should not be based on the Grade 3 label alone. A proper assessment should consider pain/discomfort, testicular size and symmetry, semen quality, fertility plans and hormonal status. In men with abnormal semen parameters, varicocele repair can improve sperm concentration/count and, in some studies, motility and morphology; pain improvement after repair has also been reported in approximately 48–90% of patients. Management of Grade 3 varicocele varies from person to person. Under the guidance of nearby qualified   Ayurvedic surgeon/physician, Panchakarma therapies-such as Jalaukavacharana (leech therapy) and Basti (medicated enema)-can be effective. In Ayurveda, this condition is correlated with Siraj Granthi or Siraj Vriddhi& is managed using classical formulations like Triphala Guggulu, Varunadi Kwath, and Vriddhivadhika Vati to relieve symptoms

Management of grade 3 varicocele is cure but without seeing it's not possible you can consult your nearby vaidya Leech therapy is best for varicocele and sirabedhan also

“In Grade 3 varicocele, Ayurveda or Homeopathy may help with associated symptoms such as discomfort or general wellbeing, but there is currently no good-quality clinical evidence that either treatment can permanently reverse or close the dilated veins. Evidence that they improve testosterone or semen parameters specifically in Grade 3 varicocele is also insufficient. If there is pain, testicular size reduction, abnormal semen analysis or fertility concerns, evaluation by a urologist/andrologist is important. Evidence for varicocele repair is stronger in appropriately selected men, particularly those with a palpable varicocele and abnormal semen parameters; studies show improvement in semen parameters in some patients, while testosterone benefits are less certain and individual. You may use Ayurveda as supportive care, but I would not recommend relying on it alone to reverse a Grade 3 varicocele.
