The combination of severe pain near the anal opening that makes walking and sitting difficult, along with fever and a burning sensation, is highly suggestive of a perianal abscess â a collection of infection/pus in the tissue around the anal canal. This is a surgical condition, and while sitz baths help with comfort and hygiene, they cannot drain or treat an abscess. A perianal abscess typically requires a minor surgical procedure (incision and drainage) to properly treat. If left untreated or treated only with antibiotics, it can worsen or progress to a fistula-in-ano (a tract that forms between the abscess and the skin surface), which is harder to treat.
Next Steps
Please see a General Surgeon or visit a hospital urgently â ideally today or tomorrow, not later. Do not delay further as you already had a day of fever suggesting active infection. Continue sitz baths for temporary comfort in the meantime, avoid sitting on hard surfaces, and eat a high-fibre diet to avoid constipation. Please consult me directly on Practo for a detailed evaluation.
Proper evaluation and personalised advice are needed for your concern. A detailed consultation would be best to provide accurate guidance.

Severe anal pain preventing sitting/walking, combined with a fever, strongly points to an anorectal abscess. Continue sitz baths, but see a surgeon immediately.
