7cm defect is large. Chances of obstruction are less, nevertheless can happen. The defect has to be closed surgically.

Umbilical hernia -if it having small defect like less that 3 cm, it will result in frequent obstruction of intestines. Very bigger defects, chance of obstruction is less, though it is there anyways. If it is cosmetically disfiguring big swelling, you can go ahead with surgery. If she is having any other significant problems like diabetes, heart disease, and kidney problems due to her age, you can decide after seeing the risk of anesthesia.

If she has vomiting, if you are not able to push the hernia inside, then it might be obstruction, then consult surgeon immediately
