Tooth infection
Yesterday I had RCT after pus was cleaned. Before the procedure, I had severe pain, a 103°F fever and high BP. I started Metrogyl 200 and Augmentin 1g twice daily. On Day 2, the pain reduced a lot, but at night it returned with a 100°F fever. Now I see new, painful pus/swelling on another side of the treated area. My dentist advised warm salt-water rinses and very gentle finger massage. Is this normal after RCT, or could it mean a new abscess or spreading infection? Is gentle massage safe? How often should I rinse? Since I had a 103°F fever yesterday and fever has returned today with new pus, should I see my dentist urgently? What symptoms need emergency care?

Hi mam After a succesful RCT the pain and fever should have completely resolved, you experiencing the above said symptoms indicates there could be leftover infection or newly formed abscess. Kindly stop the massages and rinses. Do continue the antibiotics. You should see your dentist asap. Emergency symtpoms will be Pain radiating to neck, forehead and swelling over the area and high fever.

Fever returning after RCT, with new painful swelling and pus, is not a normal recovery sign and can mean persistent infection or an abscess. Please contact the treating dentist/endodontist today for examination, dental X-ray and drainage or further treatment; do not massage or squeeze it, and do not extend antibiotics without review. Go to emergency care immediately if swelling spreads to the face, eye or neck, or if you have difficulty opening the mouth, swallowing or breathing. You may book a Practo consult with me for coordination, but dental review should not be delayed.
Next Steps
See the treating dentist/endodontist today for examination, dental X-ray and drainage or further treatment. Use only prescribed antibiotics; seek emergency care for facial/neck swelling or difficulty opening the mouth, swallowing or breathing.

Antibiotics take minimum 48 to 72 hours to act, hence relapse or new infection is possible. You can do warm  salt water rinse, 3 times daily. Get clinical examination done by your dentist.
