If you have unbearable pain in your tooth then the caries has extended to the pulp cavity. That means you require root canal treatment followed by a crown over the affected tooth. The problem of pain will be resolved.

If u don’t require filling then u may be advised extraction. But don’t be in a haste. Decision making of your treatment is vested with u. But diagnosis and treatment planning can only be executed by the doctor.
