I have severe tooth ache . I really need some instant relief. I had keterol but that did not affect my pain.

It has been an acute condition. It has crossed the threshold of medicines. Get RCT or extraction done asap.

Hi According to the X-ray, there is deep caries involving the pulp of teeth and there is infection at the base of root of the same teeth. The treatment of choice is Root Canal Treatment (RCT). There are various factors to be considered for RCT to be successful and you need to visit a dentist to know about the same. Taking pain killers is only a temporary option.

Hi! Keterol will help subside the pain but the  tablet will take some time for its action. By IOPA it's evident that the decay is deep and you need to go in for RCT / removal (if mobile) - based on the clinical observations. Permanent relief should be aimed by undergoing treatment as such tooth will cause problem once the medication is stopped.
Next Steps
Consult and discuss about the treatment options regarding that tooth with the dentist and carry on with the procedure at the earliest.

If the tooth undergoes RCT then it should be followed by a cap on it!. Take care.
