Toenail yellow colour and both side gaps

I have been suffering from toenail issue for 1 year. I also underwent a minor toenail operation few months back. Doc had cut both sides nails. Now these sider nails have grown up and there had been space between nailbed and supporting skin so regrown nails have started again going into the supporting skin so it becomes very difficult to cut the nail from there. Also the toenail has become yellowish and clourless bending towards both sides. Toenail is entirely stucked to nailbed hence it becomes difficult to cut it since it starts bleeding and painful.  I want to know the permanent solution for the same. Should I remove the entire toenail? Will it grow again after removal? Please suggest best doctor for the same and some medicine which soften the toenail.
