Tightness in side of middle back
When i laugh too hard or when move in certain directions or if something hit me.. I have felt spasm(felt like something is breaking) kind of  pain for few minutes in the circled area in the attached image (side of mid back). And it will get back to normal. Having this issue from last 4 years. But in last 3 days i am feeling continous tightness in the same region. I feel that if i do something heavy that region will break... Is it something significant ?

It could be a muscle problem which is not healing. Or it could be something inside, like lungs etc. If it's muscle issue. Get vitamin d and b12 levels checked. If low, take injectable supplement for vitamin d and b12 and your pain will go as muscle will heal. If not then consult an ortho and a physician to figure out where pain is coming from and treat accordingly
Next Steps
vitamin injections
