Tight Foreskin
I am 18,have a tight foreskin in my penis. It retracts full or partially when flaccid or semi flaccid but only the tip is exposed when erect. it pains if I try to retract it with pressure. However, I can masturbate without any problem. is this a serious problem that I need to be concerned about and seek medical attention? I dont want to circumsize.

Thank you for asking. I can understand the apprehension and worry your are through. Don't worry, nothing appears very serious. It is a treatable problem. Let us understand, what the condition is? It very likely appears to be a case of Phemosis. In Phemosis, there is a constriction at the opening of foreskin, as a result of which the skin retraction is not complete and is painful. When flaccid, the penis size is small and therefore the skin gets retracted easily. While when erect, due to enlargement of penis, the resistance to retraction is more and so it is unable to get retracted completely. In some cases where the constriction is minor, retraction of skin is possible although it is very painful. During masturbation, because we are able to control the intensity of stimulation, we are able to gain pleasure while at the time we are also able to limit the pain that is caused. So while masturbating it is not so troublesome. But yes, during real sex, it can cause you great discomfort and you may not be able to do sex. Depending upon the severity, the management can be decided. In severe cases a circumcision is indicated. In mild to moderate cases, exercises and guided masturbation techniques can prove helpful.
