Further evaluation is required of his condition for which you need to reach on a conclusive diagnosis. See a good ent specialist and once a proper screening is done and acute phase is managed, then seek ayurvedic consultation.  Right now he needs immediate intervention and proper diagnosis of his problem. As a supportive treatment make him follow HAIP DIET immediately and make him keep a specialized kashayam in his mouth thrice in a day. You can book a  consult for which kashayam to use on HAIP DIET website.

The text description indicates a male patient suffering from severe difficulty swallowing and a significantly elevated C reactive protein level of 11.7 which indicates active internal inflammation. He has already been prescribed heavy, broad-spectrum antibiotics, yet his symptoms continue to worsen. If your brother is still completely unable to swallow food or liquids, you must take him to a hospital emergency room or an ENT If you have any doubts or what to do next you can get an online consultation right now so that after taking proper medical history i could tell you every step taken after.
Next Steps
Ayurveda cannot replace emergency medical interventions like intravenous (IV) antibiotics or fluid resuscitation during an acute, severe swallowing crisis. However, once an ENT specialist stabilizes his condition and he can safely swallow again, traditional Ayurvedic treatments can be introduced to clear residual inflammation and prevent recurrence
Health Tips
This looks to me like an urgent medical issue and you must reach out to a hospital and confirm the diagnosis. Once the ENT surgeon tells the exact problem after that only we can continue treatment in ayurveda.

Based on the ultrasound report and your brother's symptoms, the main concern appears to be left submandibular sialadenitis (inflammation/infection of the salivary gland) rather than the thyroid nodule. The report shows: ? Colloid nodule in the left thyroid (TI-RADS 0): This is usually a benign thyroid nodule and is unlikely to be the cause of severe pain or inability to eat. Increased blood flow in the left submandibular gland: This supports active inflammation (sialadenitis). Reactive cervical lymph nodes: These commonly enlarge due to nearby infection or inflammation. Although the report mentions loss of hilar echogenicity, this needs to be interpreted together with the clinical examination. Reactive lymph nodes are still the most likely explanation in the setting of an acute infection. CRP 11.7: This indicates ongoing inflammation, although it is only mildly elevated. The fact that he cannot eat, has persistent pain despite antibiotics and painkillers, and is not improving means he needs re-evaluation rather than simply changing medications. Sometimes salivary gland infections are caused by a salivary stone blocking the duct, an abscess, or resistant bacteria, which require further assessment.
Next Steps
Please arrange an urgent consultation with us. A detailed examination is needed to check for: A blocked salivary duct or salivary stone. Formation of an abscess. The need for drainage or a change in antibiotic based on examination. From an Ayurvedic perspective, treatment can be supportive and may help during recovery, but if there is an active bacterial infection or abscess, it should not replace appropriate medical or surgical management. Classical Ayurvedic management is considered only after proper evaluation and once any emergency has been ruled out. For a personalized Ayurvedic treatment plan after the acute condition has been assessed, you may consult us at 9 3 1 5 6 6 1 5 6 5.
Health Tips
Encourage him to: Stay well hydrated. Apply warm compresses over the affected area. Gently massage the gland towards the mouth if advised by the doctor. Maintain good oral hygiene. If swallowing is possible, sucking sugar-free lemon drops may stimulate saliva and help if there is no complete duct blockage. Seek immediate medical attention if he develops high fever, rapidly increasing swelling, pus inside the mouth, difficulty swallowing saliva, difficulty breathing, or inability to open the mouth, as these symptoms may indicate a deep neck infection or abscess requiring urgent treatment. While Ayurveda can be a valuable adjunct in recovery. Thank you

Some detailed history like duration of swelling, presence of fever or not, or whether weight loss observed within this period or not,etc are required then the effective ayurvedic treatment can be started. As of now your reports are suggesting of infection, need few more tests to confirm its nature. For symptomatic relief you can start gargling with turmeric salt lukewarm water will be good. Besides that you can also start having Kanchnar guggulu (an ayurvedic medicine).
Next Steps
For detailed solution you can consult where after the full medical history appropriate treatment and guidance will be provided.
