Throat irritation
Throat irritation is followed by coughing, choking and breath feels uncleared. Uncomfortable; may feel I could collapse, but settle. No chest pain/wheezing. Started in Bangalore in 2020 and continued in Jamshedpur. Some days are symptom-free. Mainly mornings, before 2 PM. I often feel sputum stuck in my throat and clear it. It is thick, sticky, white and hard to expel; similar material comes out during intense coughing/choking. Episodes sometimes occur outdoors/balcony, especially near trees; I may remain fine indoors, windows closed. May be more common in cloudy/rainy/humid weather. Today I woke well; after washing my face/mouth, throat irritation started. After tea, coughing/choking occurred, then settled. Morning tooth-brushing can trigger it; night brushing usually does not. Used inhaler once. Symptoms stopped in a closed AC restaurant; unsure if AC triggers. No blood/fever/weight loss. HRCT Thorax done Sept 2020; report shared. Main concern: breath does not clear, causing choking.

Six years of throat irritation with a clean HRCT, no fever and stable weight tells me this is not a lung problem at all. What you describe - thick morning sputum, choking spells, worse in humid or dusty air, better on some days - is classic silent acid reflux irritating the voice box along with an allergic drip from the nose. Steam and gargles help only briefly because the acid and the allergy keep restarting it. Treated properly for a few weeks, most patients get real relief.
Next Steps
Take an empty-stomach acid blocker daily for 8 weeks, add a night-time antihistamine and daily saline nasal rinse, avoid late dinners, tea on empty stomach and smoking or dust exposure. Get spirometry and a simple ENT scope of the throat if the choking spells continue. You can consult me on Practo with the HRCT and I will write out a stepwise plan and review you at 4 weeks.
