Wisdom tooth needs extraction if their is any hinderence in the pathway of eruption. And attached x ray shows their is hinderence in its erupting pathway. You need to get it extracted. Because it is causing trouble. Still have queries then please consult with doctor online or offline for better guidance and treatment options. Till then warm saline water rinses two to three times a day, use mouthwash and try to maintain good oral hygiene.

If there is pain and pus in the respective tooth, you better get it removed surgically, otherwise there will be recurrent infection in the same region. Antibiotics may reduce the infection and subsequent pain for a short duration, but it will keep on recurring.

Yes you should plan for extraction, or later you can develope more complicated issues like restricted mouth opening, difficulty in chewing, extra oral swelling of face, with moderate to severe pain. The problem must have arised because you are unable to brush properly in that region as the tooth is not properly aligned. Hence to avoid future complications, it's better to go ahead with extraction unless you have any other systemic conditions or taking any other medication.
