Right upper 2 premolars are decayed. Right lower wisdom tooth needs to be removed or else atleast get right lower 2nd molar(decayed) restored asap.

You have all 4 wisdom teeth and I would advise you get the right ones - both upper and lower extracted. The lower right wisdom tooth because of its position is decayed and also caused deep decay in the next tooth. It is only a matter of time before you start having pain in that region. Once the lower is removed you should remove the upper to avoid it from growing downwards later. Also I see decay in atleast 3-4 other teeth as well.
Next Steps
You should visit a dental clinic to get the exact details of which teeth require treatment. You also seem to have alignment issues and an important missing tooth which needs to be looked at.
