As per the report, you have Cervical spondylosis with degenerative changes at multiple levels. Decision of surgery depends on 1) Clinical examination 2) MRI imaging (not merely by report) 3) And your symptomatology corelating with MRI NO detailed history regarding Pain in meck or pain radiating to your left arm/ weakness (as per your mri) was explained in your question.
