At 7 years, unclear speech and difficulty expressing properly do need evaluation. Causes can range from speech/language delay, hearing issues, oral-motor problems to neurodevelopmental conditions.
I would advise assessment by a developmental pediatrician/speech therapist and hearing evaluation if not done earlier. Early speech therapy can improve communication significantly.
