Hello, I am Dr. Romain Rajan, General Physician (Ex Govt Medical Officer at RML Hospital, New Delhi), and your case has been allotted to me. Thank you for sharing your mother’s details. I understand your concern, and I will explain this clearly and calmly. Your mother being 47 years old, with normal blood sugar and blood pressure, but having grade 2 non alcoholic fatty liver, can still develop skin allergy or itching, and this is not uncommon. Possible causes of her skin allergy • Fatty liver related metabolic inflammation Even with normal sugar and BP, fatty liver is associated with internal inflammation, which can sometimes manifest as skin itching or rashes. • Dry skin and seasonal factors Especially common around the neck, abdomen, and hands, and less on the legs. • Allergic dermatitis or contact allergy Reaction to soaps, detergents, perfumes, or clothing materials. • Mild liver related bile salt imbalance Rarely, altered bile flow can cause itching even without abnormal liver tests. • Vitamin deficiencies Vitamin D or B12 deficiency can contribute to skin problems. • Insulin resistance (early stage) Can exist even when sugars are currently normal. What you should do now • Avoid harsh soaps and perfumes • Use mild soap and moisturise daily • Keep the skin well hydrated • Ensure adequate water intake • Avoid scratching • Use loose cotton clothing What should be evaluated • Liver function tests if not done recently • Thyroid profile • Vitamin D and B12 levels • Fasting insulin or HbA1c if advised When to consult a doctor immediately • If itching becomes severe or generalized • If yellowing of eyes or skin appears • If there are persistent rashes or lesions • If symptoms do not improve with basic care In most cases, controlling fatty liver with weight management, diet correction, and physical activity leads to significant improvement in skin symptoms. For personalised one to one consultation and long term management, you can reach out to me directly on WhatsApp at 85271646 seven seven. A detailed consultation helps identify the exact cause and prevents recurrence. Wishing your mother good health. Dr. Romain Rajan General Physician
