Short tempered, mood swings, murmuring
Patient age is 54. He used to talk rubbish. He has frequent mood swings. Fight a lot. Doesn't able to understand what someone says. Can't explain things. Even can't talk properly. Get angry unnecessarily. When in good mood he is extremely good person but when in bad mood worst person. When in bad mood fights with everyone. Used to talk with himself. Facial expressions changes when mood swing happens.

Hi, Looking into the history, Patient should be suffering from Bipolar Mood disorder with psychotic features. Other medical conditions must be ruled out like brain injury , substance use or thyroid disturbances etc.
Next Steps
Patient should be brought to psychiatrist for consultation to reach to accurate diagnosis and plan of treatment. He may be benefitted with mood stabilizer and antipsychotic agents.
