If your heart rate shoots up quickly with mild exercise, and you feel chest pressure when it goes above 155 bpm, even though your old echo was normal: Most common reasons Deconditioning (loss of stamina) — even a few weeks of reduced activity can make HR rise fast. Anxiety / panic response — causes sudden HR spikes and chest tightness. Low fitness + over-focusing on HR — common when someone is anxious about heart health. Anemia, thyroid issues, vitamin deficiencies — can reduce exercise tolerance. Electrolyte imbalance or dehydration — makes HR rise quickly. Old echo is 8 months old — symptoms have changed. But chest pressure = needs re-check Because you’re getting chest pressure at higher HR, you should get: ECG Echo (repeat) TSH, CBC, Vitamin D, B12 TMT (treadmill test) — especially important since your symptoms appear only on exertion. Short advice Until checked: Don’t push beyond the point where chest pressure starts. Increase exercise slowly (start with brisk walking, not running). Stay hydrated and maintain electrolytes. When to worry If you get chest pain, dizziness, fainting, or breathlessness, get checked immediately. If you want, tell me: Your resting HR Any recent blood tests Whether chest pressure happens at rest

Hello, I am Dr Romain Rajan, General Physician. Your case has been allotted to me. I understand your concern. Earlier you were able to exercise up to a pulse of one hundred and seventy beats per minute, but now after touching one hundred and fifty five beats per minute you feel chest pressure and need to stop. Along with this, you have anxiety for the past eight months. This combination is very common and can reduce exercise tolerance even when earlier tests like echo were normal. Advices 1 Do a slow warm up for at least eight to ten minutes before starting any intense exercise. 2 Focus on breathing exercises because anxiety tightens the chest muscles and increases heart rate faster. 3 Avoid heavy workouts on an empty stomach or immediately after meals. What to Monitor 1 Chest pressure that increases with exercise 2 Dizziness or breathlessness 3 Palpitations lasting more than a few minutes Management There are three major possibilities for your symptoms 1 Anxiety induced exercise intolerance Chronic anxiety makes the body enter a “fight or flight” mode quickly. This causes • Faster rise in heart rate • Early feeling of chest tightness • Low exercise capacity Even when the heart is structurally normal. 2 Deconditioning If you have reduced workouts in the last seven to eight months, your cardiovascular endurance would naturally go down. Then heart rate rises faster even with mild exercise. 3 Acid reflux or chest wall spasm Sometimes reflux or muscle spasm during exertion causes a feeling of pressure which forces you to stop. Since your echo eight months ago was normal, the heart structure is not an issue. But if symptoms are continuing, you may need • An ECG • A treadmill stress test • Thyroid and Vitamin D level check • Anxiety evaluation to know the exact reason. Most people with your symptoms improve once anxiety is treated and a gradual exercise plan is followed. I can guide you with the correct step-by-step exercise schedule and medicines if required after knowing your routine and triggers. When to Consult a Doctor Immediately 1 Severe chest pain 2 Fainting or near fainting 3 Heart rate not coming down even after rest Every patient’s situation is unique. For accurate diagnosis and lasting relief, it is important to discuss your symptoms, stress level, sleep pattern and exercise routine in detail. You can reach out to me directly for this case and also connect with me in future for any medical consultation. I am available on Practo or on WhatsApp at 85271646 seven seven for a one on one consultation where you will receive 1 A personalised treatment plan 2 Safe medicine adjustments if required 3 Quick follow ups to track your recovery Take care, wishing you good health.
