1. Advices
If you were earlier taking Propranolol SR 20 mg once daily, then switching to Propranolol 10 mg (normal tablet) should generally be taken twice a day â morning and evening, after meals for the same total effect.
Do not break the SR tablet, because it can cause fluctuations in drug levels and uneven control of symptoms.
Always take propranolol after food and avoid suddenly stopping the medicine.
2. What to Monitor
Heart rate (should not drop below 60/min).
Any dizziness, weakness, or excessive sleepiness.
Recurrence of anxiety, tremor, or palpitations if the dose is too low.
3. Management
You can safely change from Propranolol SR 20 mg once daily to Propranolol 10 mg immediate release twice daily for gradual dose reduction.
After 2â3 weeks, if you continue to feel stable, you may taper it further with your doctorâs guidance.
4. When to Consult a Doctor Immediately
If you experience fainting, very slow pulse, or breathing difficulty.
If anxiety or palpitations suddenly worsen.
If you have asthma, diabetes, or thyroid issues (dose adjustment may differ).
