Greetings
An intermittent squint in a 2-year-old child that has persisted for a year, combined with recent eye redness and frequent watering, requires a thorough re-evaluation. The symptoms could stem from three distinct medical possibilities: allergic conjunctivitis, viral or bacterial infection, or localized eye strain and irritation related to the misalignment. Potential management and treatment options include pediatric-approved lubricating or antihistamine eye drops for the irritation, orthoptic eye exercises to strengthen coordination, or corrective spectacles if a minor refractive error has developed. Intermittent misalignments are frequently monitored via observation alone if the child maintains excellent binocular tracking and equal vision strength. However, because localized inflammation, swelling, or tracking changes can alter the care path, a follow-up assessment with a specialist is strongly advised

At this age squint needs treatment soon or the eye can become lazy (amblyopia). Redness and watering together suggest possible blocked tear duct or mild infection too. Waiting 1 year without starting treatment is too long.

Get a cycloplegic refraction done at a good pediatric eye centre â don't wait further. Consult me on Practo for more guidance.
Hello,
Based on the history you have provided, this appears more suggestive of allergic conjunctivitis. You may use Ecotears eye drops (1 drop in the affected eye twice daily) along with an anti-allergic eye drops.
I would recommend getting the eyes examined by an ophthalmologist to confirm the diagnosis and prescribe the appropriate anti-allergic medication. In the meantime, try to reduce screen time, avoid rubbing the eyes, and ensure adequate rest.
Regarding the squint, it is a separate entity. Please continue to follow the advice and treatment plan given by your ophthalmologist for the squint.
Intermittent squint in young children can sometimes be normal, and if two pediatric ophthalmologists have examined her and advised observation, that is reassuring. However, since it has persisted for a year, she should continue regular follow-up with the pediatric ophthalmologist as advised to monitor vision and alignment.
The redness and watering are not usually caused by the squint itself. They may be due to allergy, viral conjunctivitis, dry eyes, or irritation and should be examined separately.
If the redness is mild, you can use preservative-free lubricating eye drops (carboxymethylcellulose) 4–6 times a day until she is examined. Avoid using antibiotic or steroid eye drops without an ophthalmologist’s advice.
If the eye becomes painful, there is yellow discharge, sensitivity to light, decreased vision, or the redness persists for more than 2–3 days, please have her reviewed promptly by an ophthalmologist.
