My almost 3 year old son suffers from recurrent anal region rash. It is circular in shape, mild red. Every time , i apply zinc oxide cream for 5-6 days, it improves and then comes back again. He is diaper free child since one year. This pattern is recurring since many months, he is on soy milk, as he cannot digest cow milk since one year. Rest he is healthy, eats fine and weighs 14 kg at 2.7 yrs. He stool is almost fine but he occasionally has mucus in his stool. Why this is repeating?? Please, reply.

Hello,
There could be several reasons for a recurrent perianal rash. Since it keeps recurring, applying zinc oxide cream alone is likely providing only temporary relief without addressing the underlying cause.
As your child has a cow’s milk protein allergy and is on soy-based feeds, please note that a small percentage of children can also have cross-reactivity to soy. If your paediatrician has advised complete avoidance of cow’s milk protein, ensure that all dairy-containing foods, including biscuits and other processed foods with hidden milk ingredients, are avoided.
If there is also a suspicion of gluten sensitivity or celiac disease, follow a strict gluten-free diet only if advised by your treating doctor, as starting a gluten-free diet before evaluation can affect diagnostic testing.
Other possible causes of recurrent perianal rash include dermatitis, pinworm (worm) infestation causing perianal itching, bacterial infection, or other gastrointestinal conditions.
I would recommend consulting your paediatrician while the rash is present so that it can be examined directly. This will help identify the exact cause and guide appropriate treatment rather than relying only on barrier creams for temporary relief.
Hii
This needs through examination and investigations to identify the cause of rash. As you pointed out child have cow milk allergy, he can have gluten allergy as well. Ensure there is no consumption of milk.Get in touch with the Pediatrician.
Greetings
A recurrent perianal rash and mucus in stool can indicate an underlying gastrointestinal or dietary issue.
Because he cannot digest cow's milk, he may have developed a cross-reactive intolerance or allergy to soy protein.
Alternatively, a persistent circular rash around the anus could point to a localized infection, like perianal streptococcal dermatitis.
Zinc oxide cream provides temporary relief but does not treat the root cause of an infection or ongoing dietary sensitivity.
Since the rash is recurring for many months and improves only temporarily with zinc cream, it is unlikely to be just simple irritation. Possibilities include a bacterial infection around the anus (perianal streptococcal dermatitis), fungal infection, or less commonly irritation from stool.
I would recommend getting the child examined when the rash is present, as the treatment depends on the exact cause. If possible, please share a clear photo of the rash during an active episode so I can guide you better.
Also let me know if he has itching (especially at night), pain while passing stools, constipation, or blood in stools.
