Recurrent anal fissure
I have had a fissure for 1.7 years. It heals but comes back every 3 months as a small cut at the top (12 o’clock position). My motions are always soft, and I drink 2–2.5 litres of water daily. When my stool is about the thickness of 1.5 fingers, I feel the inner muscle open or stretch as the stool passes, and then contract back when it slides down. This seems to cause a spasm. How can I naturally reduce these spasms? Does gentle finger massage inside help?

For Acute Fissure in Ano, we can go for conservative treatment with & chance to complete healing *Avoid oily, spicy & Non veg/ High proteinaceous diet *Bland diet intake *Semisolid diet *Adequate quantity of water before or after food * Stool softener/ Laxatives( liquid paraffin or cremaffin gel) *Avoid sitting for a longer period  without crossing over the legs/ Avoid long travel & bike riding * Topical sphincter relaxants like Calcium Channel Blockers ( Nifedipine) *Topical soothening  agent like Sucralfate  with bactericidal ( povidone iodine & Metronidazole) e.g  Drez S ointment *Sitz bath , 2 times daily ( Warm water + Dettol or savlon or betadine solution) But in case of Chronic fissure in ano,  conservative treatment suggested only to take a break ( Temporary relief). This may recur after a week, month or years later & leads to a lot of complications like Perianal abscess,Fistula in ano & even Piles (secondary to strainful  defecation to overcome sphincteric spasm). So, in case of Chronic ( or) Recurrent (or) Acute on chronic fissure , tis better to go for surgery
