I run regularly 3-4 days a week, however I feel fatigued after workout / long runs more than 10 kms. I usually eat well, however mostly I run in fasting state. Do I need to get myself some tests done to know the cause?

What, do you expect, should happen when you run 10 km or when you do workouts ? Should you not feel fatigued?
You should know that quite a few Gym instructors have died in the Gym while they were on duty.
So being physically fit doesn't guarantee any immunity to heart attacks.
Please consult a Cardiologist for a total checkup. If you have symptoms you must visit a Cardiologist.
