RCT. X-ray added. Do I need RCT or not?
I have severe ear pain, jaw pain with swelling underneath ear for a month.went to the dentist last week after treating sinus,ear infection. He took xray, said have a crack of fracture and told I need to extract tooth.i went to Endo for second opinion.he said I don't have fracture. Might have a minor crack and have more cold sensitivity in that tooth(1st lower molar).he said I can have crown for that.i told it's very throbbing pain while lying down.he said then I have to test more and did more cold spray test.it makes my tooth area sore till now. Is it normal?he said if I have throbbing pain in the night while lying down, I should get RCT done. Do I need RCT for throbbing pain?whether sensitivity and minor crack needs RCT ? I never had any fillings or RCT before. Dr said my tooth is healthy otherwise. If it's paining, I should get RCT. But my dentist telling I have to extract. Very confusing. I am in USA. Should I go for RCT? I have jaw pain and swelling for past one month.help pls.

Your x ray shows slight furcation involvement i.e, the area where root divides and slight infection can be seen which might be a cause of your pain. But to confirm it more it's better if you take an cbct. According to that further treatment can be decided.

In this x ray your problem in lower second premolar(near to the first molar) There is clearly shows decay extend into   pulpal nerve area.
Next Steps
visit dental clinic and do rct on that second premolar. better result will be happen

Hi... Plz confirm that diagnosis with 3d scan called cbct. If your cbct scan shows crack in tooth or root... Treatment can be planned according. Take care.

Hello dear.... Yup...it looks confusing as your x Ray are not telling one story...but your clinical symptoms are telling the another story... It need a detail discussion to finalize the final treatment plan accordingly your past history...so find some time and text...

Hello practo user.... As I have gone through the x-ray it seems normal .. Nothing to be related with crack...but in relation to lower 1st molar teeth there is slight widening of space between tooth and bone...which may lead to food accumulation there...in those case it may cause  infection of the tooth. Your detailed clinical evaluation is needed . Regarding past medical conditions.

Hi , We have to agree on the fact that you definitely have infection and hence a symptom of pain and need treatment . Now looking at your xray which shows no sign of a decay a crack tooth syndrome is a possibility . I would advise you a CBCT of that region to be absolutely conclusive on the diagnosis . Having said that on the treatment front that i would strongly advise a RCT of that tooth in comparison to an extraction . As saving the tooth should always be a priority rather than extraction . Many a times a cracked tooth can be corrected by an RCT only if the crack is deep than extraction is the only option .
