The presence of a thin vascular loop near the trigeminal nerve without evidence of indentation or neurovascular conflict suggests that this vascular loop is unlikely to be the cause of your trigeminal neuralgia symptoms.Idiopathic trigeminal neuralgia is a possible diagnosis in the absence of clear nerve compression.Based on the MRI findings, there does not appear to be a significant neurovascular conflict that would need MVD. However, I need to see the MRI images personally, then only we can guide you further.

Hello, hope your pain has reduced. In case you are still suffering, before causing anything idiopathic, all efforts should be made to rule out other possible explanations ( which was done or no in your case?) there are many mimics of trigeminal neuralgia, which are to be considered in your case considering mri is normal. Detail history with nature of pain and associated symtoms will be needed.
