Go for paychiatric consultation first,after detailed evaluation of her history,psychiatrist can guide you through further treatment plan,more than that it seems you are more concerned regarding empathy and concerned behaviour towards your sister,that will be taken care too by psychiatrist during management procedures

I may need further clarity like the duration of symptoms, any (organic) physical ailment, her  intellectual status. age 22 is the general age of the start of mental illness. Before coming to any diagnosis we may need detailed evaluation (history) to rule out physical causes of her illness.
