I had COVID in 2020. After that, I started having breathing problems. I go to a government hospital regularly.
My IgE is very high – around 7000 IU/mL. Before COVID, I never had any breathing issues.
My PFT results have been decreasing over time, from 110% to now 85%.
Recently, I tested myself privately for ABPA (Allergic Bronchopulmonary Aspergillosis), and the result was 4.68 kU/L (positive according to the lab), but the pulmonologist did not recommend antifungal treatment.
He only gave LABA + Budesonide inhalers.
I am planning to consult an MD Internal Medicine doctor.
1. Should I meet an internal medicine doctor?
2. Do they understand PFT reports properly?
3. In your opinion, should I start antifungal treatment for ABPA?
I am looking for multiple expert opinions. Please guide me.

Pulmonologist is more likely to give you a better opinion than internal medicine.
If asymptomatic ABPA is diagnosed, we may or maynot treat it as it is not compulsory to give treatment of abpa as we see risks vs benefits. But ofcourse you need a proper clarification and explanation about everything and that should be done by the pulmonologist treating you. Pft reports are better understood by a pulmonologist than internal medicine although it depends on person to person basis
About antifungals to be started don’t self medicate, take proper consultation from a pulmonologist

kindly connect with your detailed reports..and antifungals are not to be started unless a detailed examination of all of your reports are done..your question should be when to start systemic steroids..you have an excessively high ige along with +ve ag..let us see your reports in detail before starting any antifungal or steroids

There is no need to panic! It is fungal infection in asthma. U need to be started on antifungal meds with asthma treatment. Ur pft and ur symptoms u mentioned are suggestive of asthma symptoms.
Pulmonologist will help u treat your condition.. not internal medicine..
Pft can be interpreted by pulmonologist..
Before starting antifungals we need to diagnose which stage of ABPA you fall into and whether u fit into criteria for starting antifungals .. all forms of ABPA do not require antifungals.. we need to stage and start accordingly
