Post acl and meniscus surgery
I am 6 months post ACL and still cannot get full extension. The MRI shows a medial meniscus tear. Do you think this is a mechanical block that requires arthroscopy, or is a short trial of focused PT reasonable?”

A meniscus tear may or may not be painful depending upon the type and size of the year. A post ACL tear can go for stiffness. Please post MRI images if possible.

At 6 months post-ACL surgery persistent loss of full extension is not typical and does raise concern. A medial meniscus tear—especially a bucket-handle or displaced tear—can act as a mechanical block, preventing full extension and often does not improve with exercises alone. However, stiffness from arthrofibrosis or quadriceps inhibition can sometimes mimic this and may respond to very focused extension-based rehabilitation. If the MRI or clinical exam suggests a locked knee or hard end-point, arthroscopy is usually required.
