Hello, both the teeth need to go for a root canal procedure with a crown and also need to check about the bone which is near to infected teeth. Pls visit the dentist as soon as possible.
Yes the infection has reached the inner most region of the tooth ( pulp) , so rct is must in 1st molar and in the 2nd molar it’s nearer to the pulp the infection so it might start as a normal filling but as we go deep and pulp involvement is found that could also need to be treated by rct.
Defending a yes to save both the molars and there is sign of boneloss in the IOPA get that treated as well to save the tooth from getting mobile and loosing them later
