But from the last 2 years, her condition changed completely:
she laughs, shouts, and hits suddenly
runs around
does not understand clothes (sometimes stays naked)
does not understand toilet (pees or passes stool anywhere)
not aware of surroundings
not behaving normally at all
Relatives and neighbours are telling us to admit her in a “mental hospital permanently” and saying she will never be cured. We are very stressed and confused.
I know you cannot diagnose without meeting her, but please tell me what this could be, so I can learn more online. We will take her to a doctor soon but we have financial problems right now.
Please guide. Any idea or direction will help us understand her condition better.
She is now 34 years old

Hello, The symptoms described- sudden laughing/shouting, hitting, running around, loss of understanding of clothes and toilet use, unawareness of surroundings, and a long period of silence earlier- point toward a severe neurological or psychiatric condition that requires a detailed in-person evaluation.
Given her history of severe headache during TB treatment, prolonged silent phase, and then a complete behavioural change over the last two years, this could be related to:
Residual effects of TB involving the brain (e.g., TB meningitis sequelae)
- Seizure-related behavioural changes
- A severe chronic psychiatric illness causing disorganisation and loss of functioning
- Cognitive decline due to a neurological condition
None of these can be confirmed online, but her symptoms are medical, not intentional, and they do not automatically mean she needs to be admitted permanently, nor that she can never improve.
Next Steps
She will likely need:
- A psychiatric assessment
- Neurology evaluation
- Basic tests: MRI brain, EEG, thyroid profile, vitamin levels (these are often subsidised).
. If there is a past TB history, inform doctors clearly; they may consider TB-related brain complications.
. Start treatment as early as possible. Many patients with similar symptoms show improvement once the correct diagnosis is made and medications/therapy are started.
. Only consider long-term admission if advised by doctors after proper evaluation- permanent admission is rarely required.
Health Tips
- Please do not listen to relatives who say she will “never get cured.” Many of these conditions are treatable, especially when properly evaluated.
- Ensure her safety at home- remove sharp objects, provide supervision if she wanders or becomes aggressive.
- Maintain a calm routine; avoid scolding or restraining unless necessary for safety.
- Recovery takes time, but getting the right diagnosis is the first and most important step.
- You are doing the right thing by trying to understand her condition- please seek medical care as soon as you can.

I’m really sorry to hear what your family is going through. I will explain in simple words.
From what you described —
• she suddenly laughs, shouts, hits
• runs around
• doesn’t understand clothes or toilet
• doesn’t know what is happening around her
• her behaviour changed completely in the last 2 years
This shows that her brain and mind are not working normally right now. This can happen because of different reasons.
Possible reasons (in simple words):
1. Serious mental illness
Sometimes people lose touch with reality. They may act strange, talk or laugh suddenly, not understand daily activities.
This can be treated with medicines.
2. Medical problem in the brain or body
Infections, hormone problems, fits (epilepsy), vitamin problems, or any brain issue can cause this kind of behaviour.
These problems can be found with simple tests.
Very important:
This does NOT mean she has to be admitted to a mental hospital permanently.
With the right treatment, many people improve a lot.
Next Steps
1. Take her to a Government Hospital Psychiatry Department
• They will check her properly
• Cost is very low or free
• They can give medicines that help her calm down and think better
• If needed, they may admit her for a few days — not forever
2. Keep her safe at home
• Stay calm when she becomes restless
• Remove sharp things
• Keep her in a safe room when she gets aggressive
• Don’t shout at her — it can make it worse
Health Tips
Please don’t lose hope.
Many people with similar conditions get much better after starting treatment.
