Persistent Sinusitis with Ear Block
I have been experiencing multiple symptoms over the past few weeks and would like a detailed evaluation and treatment plan.  - I have urticaria (hives) for the past 1 month. It comes and goes and seems related to allergy. - I have sinusitis symptoms for the past 2 weeks, including:   - Nasal blockage (both nostrils)   - White mucus   - Forehead pressure (especially on one side) - Since yesterday, I developed ear blockage/fullness sensation, especially after travelling. It feels like pressure inside the ear and head. This started after travel (car window open + yawning), and since then the ear feels blocked and uncomfortable. Current Medications: - Azeflo nasal spray (azelastine + fluticasone) - Levohal M (anti-allergy tablet) Concerns: - Ear still feels blocked despite medication - Sinus symptoms not fully improving - Ongoing urticaria - I am currently 3 months pregnant.

Ear pressure is because of sinusitis. Take steam inhalation three times a day. Valsalva maneuver 20 times a day.

Hello, your sinus symptoms are associated with your ear symptoms. You need to have your ear and nose examined . Nasal endoscopy is required. As you are currently pregnant do not self medicate

Since you are 3 months pregnant our aim is symptom control with maximum safety of the baby. Now, you mention that you have both urticaria and rhinitis symptoms, with no history of fever, your nasal symptoms are most likely due to allergy. Also the ear blockage which is accompanying is mainly due to nasal congestion causing Eustachian tube dysfunction. Normally, the medicines that you are taking would be considered first line for your condition . But since you are pregnant, I would suggest to switch to safer alternatives.
Next Steps
-Best management is to start taking steam inhalation atleast thrice a day. This would help with nasal and ear issues both. - start using saline nasal sprays thrice a day followed by budenase AQ Nasal spray after a gap of 10 minutes . - Take Cetrizine 10mg once at night before sleeping.

For urticaria consult a dermatologist. Try to rule out allergens in your surroundings and avoid them Use mask when exposed to dust.
