Period pain 1 week before
My periods are regular and I have mild symptoms of pcos. I have had acute period pain on first days of period for a lot of years but from a past few months my pain starts a week before my periods start. Is this normal? Is this another symptom of pcos or there might be some other problem? Also pain on day1 of period is so bad that meftal spas pain killer is not able to help.

Pain as you mentioned is a possible symptom of PCOS but it is not clear indicator of anything. Pain before a week of menses is sometimes normal which is known as PMS ( pre menstrual syndrome ). The underlying cause of the pain could even be Endometriosis, Uterine fibroids among others. So kindly consult a doctor of your choice so that a detail history can be taken to help determine the underlying cause of your pain. All the best.

definitely need an ultrasound scan to find out the reason for pain and if everything is normal then it can be corrected with cyclical usage of tablets

Premenstrual pain  can be due to many causes like endometriosis , PID , fibriod etc.  Get a usg done and consultant some gynaecologist
