Medically There is no specific position either upward or downward for the penis inside underwear.
Keep it in which ever position is most comfortable and does not cause pain or excessive pressure.
Wear comfortable well-fitting cotton underwear.
Avoid underwear that is too tight.
Since you are uncircumcised and has no foreskin problems circumcision is not necessary solely to keep the glans (head of the penis) exposed.
If the foreskin retracts easily and comfortably it is fine to expose the glans temporarily (for cleaning or if comfortable)
but it is generally best to return the foreskin over the glans afterward. This helps protect the glans from friction, dryness, and irritation.
Avoid using harsh soaps, antiseptics, powders, or perfumes on the genital area.
Maintain good genital hygiene.
