There is no medically correct direction in which the penis must be positioned inside your underwear. You can keep it upward, downward, or to the side, depending on whichever position feels most comfortable. The important points are that there should be no pain, excessive pressure, or bending, and your underwear should provide support without being excessively tight. Regarding your foreskin, if you are uncircumcised and have a normal foreskin that retracts comfortably, there is no need to keep the glans (penis head) permanently exposed. The foreskin naturally protects the glans from friction, dryness, and irritation.
Next Steps
If you have any difficulty retracting the foreskin, pain, recurrent infections, or swelling, consult us, for personalized guidance, you may also consult us at 9 3 1 5 6 6 1 5 6 5.
Health Tips
Wear comfortable, breathable cotton underwear that is supportive but not overly tight. Wash the area daily with water, gently clean beneath the foreskin if it retracts easily, and dry the area well before replacing the foreskin. There is no health benefit to permanently exposing the glans if you do not have a medical reason for circumcision, so it is best to allow the foreskin to remain in its natural position. Thank you

Medically There is no specific position either upward or downward for the penis inside underwear. Keep it in which ever position is most comfortable and does not cause pain or excessive pressure. Wear comfortable well-fitting cotton underwear. Avoid underwear that is too tight. Since you are uncircumcised and has no foreskin problems circumcision is not necessary solely to keep the glans (head of the penis) exposed. If the foreskin retracts easily and comfortably  it is fine to expose the glans temporarily (for cleaning or if comfortable) but it is generally best to return the foreskin over the glans afterward. This helps protect the glans from friction, dryness, and irritation. Avoid using harsh soaps, antiseptics, powders, or perfumes on the genital area. Maintain good genital hygiene.
