From last 2-3 days, I am having dull pain in testis and it's becoming more prominent when wearing slightly tight fitting clothes such as denim trousers. Pain is radiating towards the abdomen region and the left thigh region. Very frequently I also get sharp cramp like pain behind scrotum sac during urination which stays for 4-5 mins.

Your testicular and groin pain pattern is not typical of a simple strain. It suggests possible epididymal, prostate, or cord-related inflammation, which needs a physical examination for accurate diagnosis. I recommend you come for an in-person evaluation so we can confirm the cause and start the right treatment before it progresses.
The pattern of symptoms you’re experiencing —
• dull testicular pain for 2–3 days,
• pain increasing with tight clothing,
• radiation to the lower abdomen and thigh,
• cramp-like pain behind the scrotum during urination,
• discomfort lasting several minutes after passing urine
is not typical of a simple strain or temporary irritation.
This combination suggests that there may be an underlying epididymal, testicular, or prostate-related cause that needs a proper physical examination.
In many patients, these symptoms are seen in conditions such as:
1. Early Epididymo-Orchitis
(inflammation of the testicular cord or epididymis)
This causes:
• dull or dragging pain,
• radiation to the groin/thigh,
• discomfort with urination.
Early treatment prevents progression.
2. Prostatitis or Pelvic Floor Inflammation
This often explains:
• pain behind the scrotum during urination,
• cramp-like pain that lasts 3–5 minutes,
• radiation to abdomen or thigh.
This condition cannot be diagnosed accurately without a clinical assessment.
3. Varicocele / Cord Congestion
Pain worsens with:
• tight trousers,
• long sitting,
• end of the day fatigue.
4. Inguinal nerve irritation or hernia
Dull ache radiating to thigh/abdomen often points toward this.
Next Steps
Why you need to be examined in person
Testicular and groin pain has overlapping symptoms across multiple conditions, and only an in-person clinical check can confirm:
• exact site of tenderness,
• any swelling or thickening of the epididymis,
• signs of varicocele,
• presence of hernia,
• prostate involvement,
• whether an urgent ultrasound is needed.
Without examining you physically, the most important causes cannot be ruled out confidently.
Because you are already having urinary pain and radiation of symptoms, an early consultation helps prevent it from worsening.
What I will assess during consultation
• Testicular and cord examination
• Inguinal and abdominal exam
• Prostate/ pelvic floor tenderness (non-invasive)
• Need for urine test, ultrasound, or prostate evaluation
• Whether antibiotics, anti-inflammatory treatment, or supportive therapy is needed
This gives a precise diagnosis so treatment is targeted, not guesswork.
Health Tips
Why it is better not to delay
Many testicular or prostate-related problems are very treatable in the early stage, but if ignored, the pain can spread, become chronic, or require longer therapy.
Your symptoms have already started radiating, which means the underlying structure is getting irritated.
Getting examined once will give complete clarity and peace of mind.
Likely an infection affecting the testis and urinary tract. We call it ‘epididymo-orchitis’. Get urine sample tested for infection and take Ciprofloxacin 500mg capsules twice a day for 5 days with food. Should settle. If worsening see your local doctor.
All the best.

Your symptoms—dull left testicular pain worsened by tight clothing, radiation to lower abdomen and left thigh, plus sharp cramp-like pain behind the scrotum during/after urination lasting 4–5 minutes—are highly concerning for acute epididymitis, prostatitis, or a left ureteric stone, and require urgent in-person evaluation today or tomorrow by a urologist or in the emergency department. You need an immediate physical exam, urine test ± culture, STI screen, and scrotal ultrasound with Doppler (to exclude torsion); until seen, wear only loose clothing, take ibuprofen 400–600 mg if safe for you, stay hydrated, and avoid sex—these symptoms can worsen quickly and must not be ignored.
