Hi There's a deep decay in your wisdom tooth. I'd advise you to get it extracted sooner than later as the infection may spread to the adjoining 2nd molar. The wisdom tooth rarely helps in chewing, and if it's causing issues, there's no harm in getting it removed.
Next Steps
Consult a dental surgeon for further advice.
Health Tips
There will be a total down time of 2 days so better to get it removed during a weekend.

Hi the reason for your pain is the extensive decay wrt the wisdom tooth. Since its so severely decayed, it would be best to get the tooth extracted. The tooth is tilted, partially impacted(embedded in bone) and looks like its fused to the adjacent molar, but it can definitely be extracted using the right techniques.

The wisdom tooth has a big deep irreversible cavity and is partially erupted. The best solution is to get it removed surgically. Infection from this tooth has high chance to effect the adjacent tooth in form of root caries which is not visible from outside.
Next Steps
visit a dentist to get it removed as soon as possible. As taking medication will not help in the long run.
Health Tips
surgical extractions are not painful if done the right way. if your work requires a lot of verbal communication get it treated on a weekend to get ample rest. Till then keep the area clean by rinsing thoroughly with lukewarm water and salt after every meal.
