you are suffering from patellofemoral pain due to patella tilt both sides secondary to weak quadriceps muscles. there is a need to stop irritating the muscle, reduce pain by patella taping and then progressively improve the flexibility and strength of the muscles.

Your MRI shows old tear of one of the most important ligament around the knee joint (MPFL). This can lead to recurrent dislocation of patella and arthritic changes in knee joint.
