Pain involving the heel, knee and rest of the leg can have more than one cause. Spider veins with swelling may suggest venous insufficiency, especially if symptoms worsen after prolonged standing. Heel pain may also be due to plantar fasciitis, while knee pain at this age can be related to early degenerative changes or other musculoskeletal causes.
Next Steps
A clinical examination is important to check the knee and foot as well as the swelling, pulses and venous circulation. Depending on the examination, investigations may include X-rays of the symptomatic joints/heel and a venous Doppler of the lower limbs if there is significant swelling, prominent veins or heaviness. Basic blood tests such as CBC, blood sugar, kidney/liver function and thyroid tests may be considered; Vitamin D/B12 can be checked if clinically indicated.
Health Tips
Until evaluated, she can avoid prolonged standing, elevate the legs while resting and use comfortable supportive footwear. Compression stockings should preferably be started only after circulation has been assessed. If there is sudden one-sided leg swelling, severe calf pain, redness/warmth, chest pain or breathlessness, seek urgent medical attention.
