Nail clubbing.
Hi Doctors, I am a 27M single. About 5 months ago, my family noticed changes in my nails, and a physician confirmed nail clubbing. I underwent investigations including 2D Echo with Doppler, HRCT chest, ESR, hemogram, T3, T4, TSH, and LFT, and all reports were normal. The doctor mentioned it could be genetic as major causes were ruled out. On reviewing my old photos, my nails appeared normal until around 2016–2017. Mild changes began around 2018, progressed by 2020, and became more prominent by 2023, with further noticeable changes up to now. I also have external hemorrhoids since around 2020, along with mild constipation and a feeling of incomplete bowel emptying, which improves with a high-fiber diet. Could there be any underlying condition linking these issues, or can hemorrhoids/constipation contribute to nail clubbing? Do I need any further evaluation, or can this be considered benign? I am also  also feeling quite depressed due to the cosmetic changes.

Finger clubbing commonly occurs due to heart, lung, liver, bowels diseases, but can be genetic as well, as in your case. If no underlying disease is present , no treatment is needed unless the person wants it for aesthetic reasons. Maintain good nail health with proper cleaning,  moisturising.

Hello there, the above described symptoms and nail clubbing indicate towards some respiratory or cardiac or some habits like tobacco chewing or smoking, for which detailed history is needed after that a better evaluation can be done. If your physician has confirmed the sign of clubbing, they are responsible for coordinating your diagnostic journey. Since your initial heart and lung screenings were clear, your doctor should now transition to evaluating non cardiorespiratory causes or refer you to specialists who can.

You will also have to visit a gastroenterologist based on your description of symptoms as few conditions in the Gastrointestinal system can also cause clubbing.

Hi, thanks for sharing your detailed history. Based on your normal cardiac, pulmonary, and lab workup, serious causes of nail clubbing seem to have been reasonably ruled out. Hemorrhoids and constipation are not known causes of clubbing, so they are likely unrelated. In some cases, clubbing can be idiopathic or familial, especially when long-standing and slowly progressive as in your case. However, since it has developed over time (not since birth), it’s still worth a focused re-evaluation to be completely sure nothing subtle is missed.
