there are so many causes but mostly it is due severly decayed tooth. please seek a dentist immediately.

The most common cause for it is the decaying of the teeth.as you are sayung you are having pain under ear it itself suggest decaying of molar teeth or any kind of impaction. at your age chances of impaction are only a few. one more reason for pain can be the the trauma due to the chronic bite.sometimes a one or tqo teeth impinges on the opposite mucosa.that transmits pain to ear also. I suggest you to visit a near by dentist.
quite possible one of your molar tooth is decayed.. please visit the near by dentist and doctor will confirm the exact tooth. since you are 40 chances are less of impacted third molar or wisdom tooth pain. most probably its due to decay and infection in one of the molars.
