First we need to ascertain by clinical examination by a doctor whether this a kidney or back problem . If it is kidney problem then simple measures can be taken by checking your urine for infective parameters and if it is infection then it can be adequately treated with IV or oral antibiotics depending on how clinically unwell you are. Regarding spine pain at this level of spine should not be taken lightly. Get this investigated by doing an x-ray whether it is trauma or due to other causes and get the treatment accordingly by your doctor. Also do some blood Hb FBC CRP ESR and urea and electrolytes . Thanks
