Hi, I feel extreme fatigue, weakness, brain fog, low energy, mental stress, hair loss, anxiety and depression. My t3 is .92, my t4 is 9.90 and my tsh is 5.907 along with weight loss. I tried to take thyroid supplements and caused me major side effects. Please kindly advice, thanks in advance. My iron is very low , attached reports for reference.

Your thyroid is only borderline (TSH 5.9 with normal T3/T4) - that is subclinical hypothyroidism and it is not what is causing this level of fatigue. The real culprit here is the low iron: brain fog, hair loss, breathlessness on stairs and low mood all fit iron deficiency, and grade 1 fatty liver is separate and reversible. Random thyroid supplements were a bad idea, drop them.
Next Steps
Continue Orofer XT once daily on empty stomach with lemon water, not with tea or milk, for at least 3 months, and repeat haemoglobin plus ferritin after 8-10 weeks. Also get vitamin B12, vitamin D and anti-TPO done, and since you have weight loss with heavy periods possible, that needs a proper look. Book a Practo consult with me and I'll set the doses and decide if thyroid needs treatment at all.

Don't take thyroid medicine for now
Instead take coriander seeds water first thing in the morning
Your reports are suggestive of iron deficiency anemia
Start taking green leafy vegetables
For iron, take irozorb plus tablet after breakfast (avoid dairy products before and after 2 hours of this tablet) one tablet daily
Start walking for 10-15 minutes twice a day preferably before meals
Start consuming a handful of dry fruits in breakfast
