There is a possibility of degenerative changes in lower spine which might be causing pain.. Has the patient taken any treatment since 2 years.. Is there any pain in lower limbs or difficulty in walking.. Is there any tingling numbness in lower limbs..
Next Steps
I will suggest to start with hot fomentation,  use a knee support like knee cap, take analgesic tablet, take calcium and vitamin d supplement, apply analgesic ointment locally at the site if pain for 3-4 times daily..
Health Tips
Avoid heavy work,  avoid Indian toilet,  avoid forward bending,  avoid sitting down. plan to get x ray of knee and lumbar spine if not relieved..
