Loose stool
For more than 6 months, I have been passing liquid stools almost every day. At night, while sitting on a chair, I often feel as if stool is leaking out like water droplets, followed by a burning sensation. About 2 weeks ago, I had an online consultation and was prescribed Sporlac DS and Roko 2 mg for 3 days. My stools became firmer for a few days, but the symptoms returned. I was advised to repeat the medicines for another 3 days, which again helped temporarily. Even now, if I eat roti, luchi, chowmein, pasta, chicken, or even mildly spiced vegetables, I develop completely watery stools the next day. Only boiled rice and puffed rice keep my stomach normal. I also continue to have the nighttime leakage sensation. A few seconds before passing loose stool, I suddenly hear a loud gurgling sound in the upper left side of my abdomen.

Six months of daily liquid stools with nighttime leakage and burning is not simple IBS — this pattern requires thorough investigation. Two important patterns stand out: (1) You tolerate boiled rice and puffed rice but react to roti, chowmein, pasta — all wheat/maida products. This strongly suggests Gluten Sensitivity or Celiac Disease, where the small intestine reacts to gluten (a protein in wheat). (2) Nighttime leakage/urgency is a feature of inflammatory bowel disease (IBD like Ulcerative Colitis or Crohn's), not typically IBS. Loperamide (Roko) and probiotics only mask symptoms temporarily and are not a solution. Please get: (a) Anti-tTG IgA antibody + total IgA — for Celiac disease, (b) Stool routine + Giardia antigen — to rule out parasitic infection, (c) Fecal calprotectin — to distinguish IBD from IBS, (d) CRP, ESR, CBC, LFT. In the meantime, strictly eliminate all wheat/maida/roti from your diet and observe if there is improvement.
Next Steps
See a gastroenterologist urgently — 6 months of this warrants a colonoscopy and endoscopy to identify the cause. Start a strict gluten-free diet as a trial immediately. Please consult me directly on Practo for a detailed evaluation.
