Large effusion with mediastinal shift is medical emergency. Visit nearby pulmonologist for urgent drainage either by tube thoracostomy or needle drainage.

You have to do the pleural fluid drainage asap.
Please don’t spend time here but seek out a good pulmonologist and radiologist who can drain the fluid.
It’s a medical emergency.
